Forms & Required Documents

Welcome and congratulations on your acceptance into the PGME training program with Shared Health. As a trainee, you are an employee of Shared Health and are required to complete the following forms to initiate payroll and benefits.

Note:
A number of the forms ask you to provide an Employee Number but you can bypass that box
and continue to Complete and Submit the form.
(Employee Numbers are issed After you are enteredon the payroll system.)

Information Action
How to create an epost account for information & action
Shared Health Account & Access Request please fill out the authentication questions and answers and return to Digital Health as instructed on the form
PGME Immunization Letter for information
Immunization & Communicable Disease Record complete & return to appropriate OESH site
OESH Confidential Health Questionnaire for information & action

Direct Deposit Form complete & send along with a void cheque
Federal TD1 Tax Form complete & send
Provincial TD1 MB Tax Form complete & send
Employee Master Data Change Form use this form if you need to update your contact information after you havfe initially registered with CPGME
Benefits Forms Action
HEB Healthcare Plan Enrolment complete & send
Blue Cross HSC Dental Plan Enrolment complete & send
Blue Cross EAP Enrolment complete & send
